What to bring

When you first arrive you will be need your insurance card, picture ID, as well as your copayment or deductible payment. Please also bring a complete list of your medications and  a doctor’s referral if you have one.  If you do have copies of any pertinent diagnostic procedural results (i.e. X-ray or MRI report) bring them also.  You will be asked to complete several forms. This usually takes 20 minutes.  Most of these can be completed ahead of time if you download them ahead of time using the link below.  

What to expect

The physical therapist will meet with you to ask you some specifics regarding your problem area or injury.  After this the physical therapist will perform a physical exam of the area.  You will need to put on a gown and/or shorts to enable the therapist to see and examine the area.  You are welcome to bring in your own loose-fitting shorts or yoga pants if you don’t want to wear the ones we provide.  The physical therapist will discuss with you the results of your physical exam and explain how your course of treatment will proceed.  Treatment will begin this first day unless we are restricted by your insurance to only do the evaluation until further treatment is approved. Expect this first visit to be approximately 1 ½ hours. Subsequent visits will last approximately one hour.
